1. Each family must complete the online enrollment form. Extended family members ordering separately must also complete the online enrollment form.

2. All orders must be accompanied by full payment. We prefer you pay using the Bank Account ($0.15 fee) or credit card and NOT by check. Checks should be made payable to ZBB. Please do not send cash. RaiseRight payments are not tax deductible because you receive dollar for dollar value.

3. If your payment is returned because of non-sufficient funds (NSF), you will be charged the cost of the RaiseRight purchase plus $30 fee payable to ZBB. After two NSF's are tendered on your account, your RaiseRight ordering privileges will be limited to money orders only.

4. The ZBB Treasurer will provide a summary of each student's account upon request.

5. RaiseRight gift cards are purchased on your behalf and are not returnable.

6. When you pick up your RaiseRight gift cards, open your order and verify its accuracy. Your signature on your order form indicates you have received your order in its entirety.

7. RaiseRight gift cards are the same as cash and should be handled accordingly. ZBB will not be responsible for gift cards that are lost, stolen, or misplaced while in your possession.

8. In order to comply with Federal Anti-Money-Laundering laws, the monthly order for each family may not exceed $9,999.

9. You must complete the WAIVER OF RESPONSIBILITY below before gift cards will be released to your student.

10. When placing an order, please note: Rebate amounts, less a 5% handling fee, will be the amount added to your student account.

11. This enrollment form will be in effect for as long as you are participating in the ZBB RaiseRight program.
