DDIX Pond Hockey League has TWO options for the 2023/2024 D&D Pond Hockey Leagues. Please fill out the form below to join your Wednesday night team's roster and/or the Thursday night King of the Hill league.

WEDNESDAY LEAGUE
Select the team you will be participating with in the Wednesday night D&D Pond Hockey League. If you are not on a team, but would like to participate please select the WAIVE WIRE option.

Team Fee: $50/night (pay on game day), $50 Deposit (due before your first game)
THURSDAY LEAGUE
You understand that you are signing up as an individual for this evening; and will be placed in a random playing rotation weekly.

 Player Fee: $10/night (pay on game day), $30 Deposit (due before your first game)
WEEKLY POP ROTATION
THURSDAYS ONLY . . . YOU WILL PROVIDE ONE (1) 12 PACK OF "POPS" AS PART OF YOUR LEAGUE FEES ON YOUR DESIGNATED NIGHT. YOUR "POP NIGHT" WILL BE PROVIDED WITH THE WEEKLY PLAYER ROTATION LIST. IF YOU ARE UNABLE TO PROVIDE "POPS" ON YOUR NIGHT, PLEASE LET US KNOW.
LEAGUE FEES
YOU AGREE TO PAY ANY AND ALL LEAGUE FEES IN FULL TO YOUR CAPTAIN AND/OR FTK ICE CENTER WHEN REQUIRED. FAILURE TO DO MAY RESULT IN YOUR REMOVAL FROM CURRENT AND FUTURE AEA EVENTS.
DEKES & DANGLES AGREEMENT
I ACKNOWLEDGE THAT ICE HOCKEY IS A ROUGH AND PHYSICAL ACTIVITY, AND THAT I COULD BE INJURED WHILE PARTICIPATING IN A HOCKEY TOURNAMENT. IN RETURN FOR BEING ALLOWED TO PARTICIPATE IN ANY AEA EVENT/TOURNAMENT, I HEREBY RELEASE AND AGREE TO HOLD HARMLESS FROM ANY AND ALL LIABILITY, AN EDGE ABOVE, FOR THE KIDZ GYMNASTICS, THE SILVER PUCK CHALLENGE, HOCKEY HOLD’EM AND DEKES & DANGLES TOURNAMENTS, JOSHUA VANCE, ALISA VANCE AND BRENT A. BELANGER, ANY AND ALL ASSISTANTS AND ITS PROMOTERS FROM ANY AND ALL LIABILITY FOR PERSONAL INJURY, DEATH, PROPERTY DAMAGE OR THEFT OF PROPERTY ARISING OUT OF MY PARTICIPATION IN ANY AEA EVENT/TOURNAMENT.
