COVID-19 Screening
Please answer all questions honestly. There are no right or wrong answers, and there will be no penalty or fee for appointments cancelled due to COVID. You will be contacted should an appointment need to be rescheduled. Your therapist has also self-screened prior to each treatment, and all patients should assume that the therapist is not experiencing any symptoms or has had any risk of exposure on the day of the appointment if treatment is offered. As always, strict infection prevention and control protocols are being maintained, including use of government-approved PPE devices, disinfection of surfaces and constant room ventilation.

If you feel that you would like to reschedule your appointment due to uncertainty or as a precaution, please contact your therapist.

As always, your privacy will be protected and the information collected in this form will remain confidential.
If you do present with any symptoms, your treatment cannot proceed and you will be asked to contact your healthcare provider or Telehealth.
