Yo quiero que mi estudiante asista a la escuela EN PERSONA. Leí y entiendo el plan de apertura de LEEP Academy y que las clases en persona se llevarán a cabo de lunes a viernes, de 7:45am a 2:30pm, o 1:20pm los miércoles y otros días marcados como "salida temprana", de acuerdo con el calendario escolar 2020-21 de LEEP Academy. Me comprometo a adherirme a este módulo de aprendizaje hasta el 27 de noviembre del 2020.

Yo quiero que mi estudiante asista al módulo de APRENDIZAJE A DISTANCIA porque existen vulnerabilidades médicas o preocupaciones relacionadas con el COVID con mi estudiante y/o en nuestro hogar. I want my student to ATTEND SCHOOL IN-PERSON. I have read and understand the LEEP Academy opening plan and that in-person classes will be held Monday through Friday, 7:45 am to 2:30 pm, or 1:20 pm on Wednesdays and other days marked as "early dismissal," in accordance with the LEEP Academy 2020-21 school year calendar. I agree to adhere to this learning module until November 27, 2020.

I want my student to attend the DISTANCE LEARNING module because there are medical vulnerabilities or concerns related to COVID with my student and / or in our home. I have read and understand the LEEP Academy opening plan, that subject schedules will vary in distance learning, but all students will receive substantive instruction consistent with the New York State academic standards and dual language immersion instructional model from LEEP Academy. I agree to adhere to this learning module until November 27, 2020.
