Have you worked as a Volunteer Usher for The Provincetown Theater before? If so, when? *
Have you received at least one dose of a COVID-19 vaccine? Please elaborate if necessary. Volunteer Ushers must show proof of vaccination upon reporting to the Theater for their scheduled shift. *
Volunteers must arrive 60 minutes before showtime, greet patrons, check vaccination cards, pass out programs, help individuals to their seats, and tidy the auditorium after the performance. Do you agree to handle these tasks to the best of your ability? *
Are you Disabled and/or are there any additional accommodations that you will need in order to work a volunteer shift? If so, please explain.
