For each 100-hour Community Health Worker course conducted by the Southwest PA Area Health Education Center there are a limited number of scholarship opportunities. Each scholarship covers the tuition cost of the training classes for one person (tuition includes course fee, all materials, CPR/FA and Adult or Youth MHFA Certifications).

INSTRUCTIONS FOR APPLICATION
1. Only complete applications will be considered. Please do not leave questions blank.
2. If you have completed high school, a GED, or some college please attach a copy of your transcripts and/or GED. 
3. There is an essay portion to this application. Please keep responses to each question between 200-300 words:

- What skills do you think would be most important for someone working as a CHW? Please elaborate.
- What makes you angry/frustrated/annoyed?
- What do you do when someone comes to you with a problem?
- Who is your favorite leader?/Who inspires you? Why?
