MMEA 66th All-State Music Festival and Concert: School Appointed On-Site Chaperone Registration
READ THIS INFORMATION BEFORE FILLING OUT THE FORM:

• This form must be completed prior to the start of the festival on March 5th, 2020.

• At least one adult chaperone MUST accompany students from your school accepted to the MMEA All-State Music Festival. Schools may provide one chaperone for the entire festival or may opt to have different chaperones at different times during the festival.

• The designated evening chaperone MUST stay overnight at either the Seaport Hotel or the Renaissance Hotel on March 5 and 6, 2020.  

• Please complete ALL information on form below.

• Before submitting, you will need a Principal E-Signature.

• You may edit the form after you submit.
