Please complete and submit this form within 72 hours of attending Friends Fest organized by Partease Productions at Phillies Bridge Farm Project on Saturday Sept 25th 1 PM to 1:30 AM and/or Sunday Sept 26th 11 AM to 7:30 PM.

You will NOT be able to attend if you do not fill out this form in advance.  

PLEASE NOTE: One form must be filled out per household. Please List the FIRST & LAST names of all attendees in your party including children.

Partease Productions, LLC & Phillies Bridge Farm Project assumes no responsibility for the health of those in attendance in relation to COVID-19. Participants may attend at their own risk, so long as they adhere strictly to the following safety protocols.
1. Wear a mask or face covering when you are within 6 feet of attendees not in your pod.
2. If you, or anyone in your household, exhibits any symptoms of COVID-19 or has been exposed to COVID-19 in the past 10 days, please DO NOT ATTEND.

Safety Protocols Agreement & Covid-19 Waiver of Liability *
By selecting "I agree" below, you are agreeing to the following statement: "I am aware of the existence of the risk in attending this event and my participation in the activity that may cause injury or illness such as, but not limited to COVID-19. I have not experienced symptoms of fever, fatigue, difficulty in breathing, or dry cough or any other symptoms relating to COVID-19 or any communicable disease within the last 10 days. I have not been, nor any member(s) of my household, diagnosed to be infected with COVID-19 virus within the last 10 days. I confirm that everyone in my household who is planning to attend, has reviewed and/or been informed about Partease Productions & Phillies Bridge Farm's safety protocols (above), and we each agree to adhere to them strictly."
ASSUMPTION OF THE RISKS AND RELEASE & INDEMNIFICATION.
I recognize that there are certain inherent risks associated with the above described activity and I assume full responsibility for personal injury to myself and (if applicable) my household members, and further release and discharge Partease Productions & Phillies Bridge Farm Project for injury, loss or damage arising out of my or my household's use of or presence upon the facilities of Phillies Bridge Farm Project, whether caused by the fault of myself, my household, Phillies Bridge Farm Project or other third parties.

I agree to indemnify and defend Partease Productions & Phillies Bridge Farm Project against all claims, causes of action, damages, judgments, costs or expenses, including attorney fees and other litigation costs, which may in any way arise from my or my household's use of or presence upon the facilities of Partease Productions & Phillies Bridge Farm Project.
