Please read these instructions carefully before completing this Personal Data Change Request Form for Access/Correction of Personal Data ("Form").

This Form is to be used by individual requesting to access and/or amend their personal data held by or under the control of ISS International School Pte Ltd ("School").

For request of Academic Documents such as Certificate of Enrolment, Student Report Cards etc, please click on the URL link here.

You are not entitled to access and/or amend data which is not personal data, or personal data not belonging to you. It is an offence under Section 51 of the Personal Data Protection Act 2012 and GDPR to obtain access/make a request for correction to the personal data of another individual without the authority of that individual. Please complete the identification box below clearly, and in detail.

To prevent improper access of the Student's Personal Data, please send an email with a copy of the Student's identification documents [either ISS Student's ID, passport or National Registration Identity Card (NRIC)] to the following email address: dataprotectionofficer@iss.edu.sg

Do indicate the name of the student in the email subject.
Note: One Form can only be used to update the personal data of one Parent, PAA, Guardian, or a Student. For multiple corrections / updates of many individuals, please create another form and submit again.
