Bus Transportation Request
Please complete and submit the following form regarding Regular, ELL or GATE Bus transportation for the 2023-2024 school year. Please provide a parent/guardian email address to receive a confirmation email message after submitting the form. 

THIS FORM IS NOT NECESSARY FOR SPECIAL EDUCATION STUDENTS WHO QUALIFY FOR DOOR TO DOOR TRANSPORTATION.  THIS FORM IS ONLY FOR STUDENTS THAT RIDE ON BUS STOP BUSES.

Pre-School transportation is only provided to Special Education students and therefore does not require this form be completed.

Once we have determined eligibility and assigned your child to a bus stop, you will be notified by email.  You will also need pick up your child's new bus ID at their school before they can ride. Please wait to be notified by your school as to when tags can be picked up.
