AFN Project & Service Request

Welcome to the Academy of Forensic Nursing Project and Service Request Form!

The purpose of this form is to request projects or services that will be produced by the Academy of Forensic Nursing. Upon submission of this form, your request will be reviewed and submitted for consideration by the AFN Executive Board.

Projects will be completed as they are approved by the Executive Board. Reviews of submissions are conducted every two weeks. If you need expedited review or have any questions, please contact us at: afn@afnmail.org

Once approved by the Board, please allow 8–12 weeks per project or service request.
