One form for each child needs to be completed.  Please complete all sections in full to expedite your registration and for tax receipt purposes. Payment is due immediately upon completion of this form. 

 E-transfers can be made to treherneaquatic21@gmail.com with a password of pool21. 

Cheques are to be made to Tiger Mountain Pool Inc. Mailing address is PO Box 56, Treherne, MB R0G2V0. If e-transferring, please ensure that your mark down your child(rens') names in the message field for easier tracking.

Morning sessions will take place from 9:00 AM to 12:00 PM, while afternoon sessions will be held from 4:00 to 6:00 PM. Confirmation of the designated swimming time will be provided up to one week prior to the start of the lesson.
Level Requested *
 It is up to the registrant to review the swim level transition chart to decide the proper level that your child should be registered in according to Lifesaving Society.  Cost is in brackets. Bronze Medallion/Bronze Cross may be offered depending on instructor availability.

Cancellation Policy *
Refunds will only be given if the cancellation is due to illness and a doctor's note will be requested. An administration fee of $10.00 per lesson cancellation will be charged.  Please check that you have read this policy.
