Alumni Nomination Form 
The Board of Trustees is looking to honor alumni during their meetings. Community members and employees are encouraged to submit candidates. Central Unified graduates serve as role models through their considerable accomplishments and contributions to the advancement of the district’s goals. We look forward to celebrating them. 

Who Can Be Nominated? 
This award is open to all Central Unified graduates living or deceased who received a diploma or certificate of completion.

Criteria: 
An individual who is nominated should demonstrate in an outstanding manner that they have contributed to Central Unified School District’s mission and vision. We are looking for nominees who exhibit the following: 
  1. Unusual or outstanding characteristics.
  2. Acts of service and dedication to the Central community and/or district.
  3. Efforts to promote better education for students.
  4. Volunteer work that betters the community or district.
  5. Outstanding work locally, nationally or internationally
