St. Columbkill Youth Ministry has put in place preventative measures to reduce the spread of COVID-19; however, we cannot guarantee that you or your child(ren) will not become infected with COVID-19.

I acknowledge the contagious nature of COVID-19 and that my child(ren) and I may be exposed to or infected by COVID-19 by participating in youth group activities and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at St. Columbkill may result from the actions, omissions, or negligence of myself and others, including, but not limited to, parish employees, volunteers, and program participants and their families.

By signing this agreement, I also understand the procedures and protocols of the St. Columbkill Youth Ministry.
-I will take my temperature BEFORE coming to a meeting.
-I will not attend meetings if I am feeling sick.
-I will wear a mask at all times
-I will sign in at every meeting stating that I am attending a meeting without any symptoms of COVID-19
-I will follow all other procedures set by the Core Team.
-I understand that procedures may change.

**Waivers will be good for ONE year... September 2020 to August 2021. You only need to fill this out ONCE.**
