2023-2024 Darby's Dancers Application

Please complete this application with as much detail as possible. Your answers will help us become better acquainted with your child, learn about their needs, create personalized lesson plans, and best align them with a UDA Darby’s Dancer Buddy/Youth Mentor for the dance year. Buddies/Youth Mentors consist of teens in 7th-12th grade (who have dance training and/or leadership experience at their school )AND Service Missionaries currently serving in the community.  

These volunteers have a desire to serve, uplift, befriend, and make a difference in the world and lives of others. Their responsibility is to help your child find success in the classroom through listening, following directions, increased coordination, building friendships, and by providing them safety and encouragement through their enthusiasm and praise. However, Buddies/Youth Mentors are not special education experts, or equipped to work with individuals with severe behavior and/or physical disabilities. Those participating in the Darby’s Dancers Program must exhibit basic verbal/nonverbal communication skills, self-control and appropriate social behavior for their age and developmental disability.

Our greatest desire is to provide dance education to as many youth/teens with special needs as possible, however, there are limitations to enrollment.  Your child may have specific health requirements or behavioral needs that our program is unable to support at this time. Through this application, your child’s potential participation, safety, and overall experience in Darby’s Dancers will carefully be considered and reviewed prior to their acceptance.  Additionally, a personal discussion may also be needed to determine if your child’s needs can and will be supported through this program

Parent Commitment to Dance Season Expectations

I understand that UDA’s Darby’s Dancers Program is in high demand as it provides weekly dance classes, one-on-one mentors, outside activities, dance costumes, and performance opportunities to youth/teens with special needs at no cost to their parents. 

I further understand this program and the dancer’s success is equal only to the parental dedication, commitment, and support that is provided. I know that much is expected from those who are afforded this opportunity. As such, I’m committed to the items below and understand that my lack of commitment in these areas may cause my child to be dismissed from the Darby’s Dancers program and replaced with a dancer on the waitlist at any time throughout the season. 

*I will attend a Mandatory Parent Meeting on Friday, September 15th from 6:00-7:00 pm at UDA’s South Jordan Studio Location.

*I will check and thoroughly read all email correspondence throughout the year.

*I will provide my child transportation to their scheduled dance class each week.

*I will ensure my child attends and arrives at their weekly dance class on time. 

*I will prioritize my child's schedule to attend to 3-4 extracurricular activities scheduled outside the classroom. 

*I commit my child’s attendance and participation in two end of year performances: UDA’s recital - June 12th - 15th (exact date/time TBA) and Utah’s Special Olympics Dance Competition in April (exact date/time TBA).

*I will provide my continual parental support throughout the year.

PHOTO, VIDEO and MEDIA RELEASE STATEMENT/AGREEMENT: I authorize and grant Utah Dance Artists the right to retrieve, reproduce and publish photos, sounds, and video images of my child participating in UDA’s Darby’s Dancers Program for advertising and marketing purposes. Furthermore, I waive the right to receive compensation from Utah Dance Artists upon using and/or copyrighting the above materials.
