Proposal Description:  Write 1-2 paragraphs giving the reader a detailed description of the program/research that will be funded if grant awarded. *
State the purpose of your proposal in 25 words or less. *
What is (are) the intended outcomes of this project?  State no more than two. *
State the intended audience and/or communities to be served.  If you have data on how many, that is useful to the reader, ie. "700 students who attend physical education at this school." *
Budget:  Please specify how funds will be used and include the quantity, cost per item, and total cost of each item purchased.  Note:  The award is $500.  If your request exceeds $500, state source of additional funds. *
FINAL STEP:  To complete your application, please have your administrator send an e-mail to Tahperd at tahperd.ed@aol.com indicating that you are eligible to receive the TAHPERD grant.
