IMPORTANT REMINDERS:
  1. This Public Training is only open to the first 100 RegistrantsStrictly NO PAYMENT, NO REGISTRATION
  2. Only one attendee per google form will be allowed. Entry of multiple participants per form will render the form INVALID. 
  3. eForm will automatically close once the maximum number of participants have been met;
  4. Successful Registrations will receive a confirmation email within 24-48 hours. Advance copy of the Invoice is available upon request otherwise, Invoice may be claimed at the venue. Payment Options are indicated above;
  5. As outlined in the implementation of CPD Accredited Training Programs, only those participants who were able to attend the webinar on the entirety of its schedule will be able to get the full CPD units;
  6. CANCELLATION TERMS: All registrations within seven (7) working days before the seminar are considered final and chargeable to the reserving party (individual or company) with or without the actual attendance of the participant/s. In lieu of cancellation, the reserving party is instead encourage to send a replacement for the non-available participant/s and advise DMD at least one day before the seminar date. You are deemed to have read and understood the cancellation terms and conditions upon submission of the registration form.
