Welcome to the 2021-22 Coach Evaluations. The following form is designed to allow you to provide feedback regarding your experiences with your player's coaching staff. Most of the questions will be via a scale (1-10) with some questions allowing you to provide some text-based feedback. This is not an opportunity to rant about your coaches, please be respectful and if you are providing negative feedback, ensure it is constructive as this can be an opportunity for these coaches to understand their weaknesses to improve.

We realize that some groups/teams have a lot of coaches, we ask, that at the minimum, you evaluate your head coach - these individuals will have (HC) beside their name. Assistant coaches are to be evaluated at your discretion.
