TERMS OF AGREEMENT: Checking the boxes below serves as my signed agreement to and understanding of all of the requirements and statements as indicated in Gregory’s Gift of Hope’s Pet Food Pantry & More program guidelines. *

I understand that the Pet Food Pantry is NOT a long term but a temporary solution to provide food for my pet(s).

My pet(s) are part of my family with inside shelter and maintained in a healthy environment and living conditions.

I agree that all pets receiving aid are for personal companionship and not breeding, profit or sport.

I agree to NOT take on any additional animals while receiving assistance.

I agree that I am picking up for my household and not for anyone not living in my household.

I understand that all food is donated so quantity is subject to change and the amount I receive could change depending on supply on hand.

I understand the food provided is limited to what is donated and may not be my pet’s current brand and that the Pet Food Pantry cannot accommodate special diets.

I understand that all pet food and supplies are donated and may not be the current brand I use to feed my pet(s), which may upset my pet’s stomach.

I agree not to resell, redistribute or attempt to return to a store, any food or care items received from GGOH's Pet Food Pantry

All the information I provided is true.

I agree to release, indemnify and hold harmless GGOH, its directors, agents and employees from an against any and all loss, damage, claims, liability, costs and expense, of any nature whatsoever, including without limitations, medical/veterinary expenses, attorney’s fee and disbursements, arising out of or related to the services, food, and products provided.

Some bags that have been donated are open, but inspected well. We request that you check each bag of food/cans/treats well before feeding to your pets.
