Bu formda öncelikle Girişimcilik hakkında ders videosunu seyretmeniz ve ardından bu ders hakkında 8 soruluk bir kuizi cevaplandırmanız gerekmektedir.

Bu kuizden en az 40 puan almanız gerekmektedir. Eğer 40'ın altında puan alırsanız forma tekrar giriş yaparak tekrar kuiz sorularını cevaplayabilirsiniz.
