THIS FORM IS FOR NEW ELEMENTARY AND MIDDLE SCHOOL STUDENTS ONLY that reside in Clark County, Nevada. Odyssey Charter Schools is unable to enroll students that live outside of Clark County.

Thank you for your interest in Odyssey Charter Schools. Complete this form to begin your enrollment process. A new form is needed for each additional K-8 student you would like to enroll in Odyssey Charter Schools. A packet will be mailed to you along with a postage paid return envelope.

In order to enroll students, the following documents are required. Photocopies of the documents are acceptable.
1) certified birth certificate
2) immunization record
3) parent ID
4) transcript (middle school students)
5) latest report card
6) 504 Plan with testing accommodations (if applicable)
7) IEP/MDT/Eligibility Report (for Special Education students)

The enrollment process also requires an online registration in Infinite Campus. Instructions for the Infinite Campus Online Registration using the Parent Portal are included in the packet. Please do not return your application until the Online Registration in Infinite Campus (OLR) is complete and you have written the OLR number in the spot indicated on the cover page. Infinite Campus OLR opens on April 16th. Applications for all new students will be reviewed for completion. We cannot accept incomplete packets or packets with missing information. Completing the process DOES NOT guarantee enrollment.

PLEASE DO NOT SEND APPLICATION BACK UNTIL AFTER THE OLR (Infinite Campus) IS COMPLETE. Our offices are closed and staff availability is limited. Please be patient as applications may take additional time to receive in the mail.
