This form must be completed by a legal guardian.

Club Membership for any player under the age of 10 who won't be playing in the Badger/Beavers Junior sides.

Club membership entitles the player to:
- Attend winter training sessions between 1st September and 31st March.
- Attend summer hockey activities between 1st April and 31st August, subject to an additional fee.
- Play U10 games, subject to an additional match fee.
- Play in England Hockey Competitions

From this season, our insurance only covers players for 3 taster sessions (training/friendly games), after which, lack of payment of fees will result in our refusal to allow the player to participate in club sessions. Players will be ineligible for league games without membership.

By completing this membership form you confirm that the player will be bound by the player code of conduct and the club constitution. Failure to follow club regulations may result in the cancellation of membership following the process detailed in the constitution.
