2024 Community Well-Being Challenge Registration
Welcome to the FREE community-wide challenge to thrive! The Challenge is open to everyone. Please complete one registration form per family member.

Starting January 2024 

Register at any time and complete six well-being activities hosted by Thrive Umpqua and our community partners by December 31, 2024! A list of qualifying well-being activities is available on our website at www.thriveumpqua.com and will be shared periodically via email, text messages and social media throughout the Challenge. 

Participants will: 
-Receive a free Thrive Umpqua t-shirt while supplies last 
-Be eligible to win monthly prizes and enter to win GRAND prizes at the end of the challenge
