This form will be used by parents to declare their intention for their children to remain in remote learning or to return to in-person instruction on Monday, October 19th. Please submit a form for EACH STUDENT. This form is due on Monday, September 28 to allow us time to re-arrange classrooms and get materials ready for your return!
