  5. 1 piece of documentation from this group:  Cell/Phone Payment, Insurance Payment, Voter Registration, Utility Payment
  6. 1 piece of documentation from this group:  a copy of a driver's license/Michigan ID from Mother, Father, Guardian, Relative or Court Appointed person
  7. Last years W2 for anyone in the home earning an income or 2 most recent paystubs for anyone in the home earning
