Country Block Party 21+ Wristband Sign-Up

The Social Committee is excited to host the Country Block Party on Saturday, June 10 from 7-10pm. To help us gauge participation from our 21 and older neighbors, we are asking that if you would like to receive a wristband for the adult beverages, please complete this form. Drinks will be while supplies last. Please feel free to bring your own food and drinks as well!

This is an individual form and each adult above the age of 21 needs to complete the form to receive a wristband. If you do not have a wristband at the event, you will not be served adult beverages.
