MSCU Primary Re-Enrollment: Program Choice
Dear Current MSCU Families,

Thank you for your patience as we completed Step 1 of our re-enrollment process (Notification of Intent to Re-Enroll). You are receiving this form because you are a family that indicated you intend to re-enroll for next school year. We are now beginning Step 2 of the process: Program Choice.

Below you will find a few questions asking about your child and the program(s) you wish to choose for next year. Please respond to the form completely and submit. You should receive an automatic response receipt in your email once the form has been submitted; this is your confirmation that we have received your program choices.

We will need your responses below, as well as the paid enrollment fee, by MARCH 8, 2019 in order to continue to hold your child's spot in the program. The Enrollment, Supplies, Special Events and Field Trips fee pays for special events, field trips and admission fees for all the fun events that our students enjoy year after year. The fee also covers all supplies and materials used by students, each family's Montessori Society dues, and state-mandated vision and hearing screenings, as well as many other logistical costs required to maintain a high-quality environment for our students to learn in.

The fee is $400 for the first child you enroll and $300 for each additional child you are enrolling at MSCU in the same school year (regardless of program).

IMPORTANT: Primary students who are re-enrolling to go to Kindergarten this fall must fill out a DIFFERENT form. If you have received this form in error, you may find the correct form to re-enroll for Kindergarten at: https://goo.gl/forms/TFV7ElgIXyxuNsYl2.

FYI: Step 3, the final step in our re-enrollment process, will consist of us generating your official enrollment contract for next year based on your program choice(s), along with a summary of the current family information we have on file for you. These will be sent to you via email or postal mail, according to the preference you marked during your notification of intent to enroll, along with instructions for any additional documentation we may need to make your re-enrollment for next year final.

** Please have only ONE adult in the family fill out the form for your child. If you have more than one child that is applying for the Primary Program, please use a different email address below when filling out each child's application. **
