DOUGLAS SCHOOL LUNCH PROGRAM SURVEY
The Douglas School Lunch Program is seeking information to determine if there would be enough interest in a Summer Feeding Program.  This program would provide 7 free breakfasts and lunches per child ages 0-21 on a weekly basis.  Please complete the following survey by Friday, May 28, 2021 so that we may make a determination and plan accordingly.  We will let families know prior to the end of the school year if we will be providing the Summer Feeding Program.  A sign up would be required for each week to participate. If it is not feasible to provide the program due to insufficient interest, information will be made available to families regarding other food resources and programs within the area.

If we do provide the Summer Feeding Program, residents will need to pick up the meals.  A variety of breakfast and lunch meals will be provided which will include fruits, vegetables, and milk. Are you able to commit to picking up the meals at a designated time and location during the day?
