I, the parent/guardian of the registrant, a minor, by checking the box below, agree that the registrant and I will abide by the rules of the league and its affiliated organizations and sponsors. Recognizing the possibility of physical injury associated with basketball, I hereby agree to release, discharge, hold harmless and/or otherwise indemnify Otsego Youth Basketball and its affiliated organizations and sponsors, volunteers and officials, including the owners of the gym and facilities used for the basketball programs, against any claim by or on my behalf of the registrant as a result of the registrant’s participation in the basketball program and/or being transported to or from the same, which transportation I hereby authorize.  I also give consent for players name and photo's of player to be published on the Otsego Youth Basketball Facebook page and other social media outlets.  No personal information about the player will be available.  Pictures of the basketball camp will be shared on the site as well.  I agree to adhere to the Ohio Department of Health rules regarding infectious disease. *
