Permissions and Payment Paperwork for June 13-16, 2023 UWGB/ANST Camp
This form gathers all the formal permissions and commits to camp payments.  The Camp Cost for food and lodging, admin fees and parking fees and Coaches costs is no more than $200.  This is payable to ANST because Coach Scott is doing the admin/treasury work for this camp.  @Scott_Putman at venmo, Paypal cputman@new.rr.com or a check on the 13th is Ok.      The camp starts on the Afternoon of Tuesday June 13 and finishes on the afternoon of June 16.  The camp outline, what to bring, final cost (subject to change) will be shared seperately.  Sign up here by May 15th please!

We are sleeping in tents in car camping fashion.  ANST/Putman has two 4 man solid backpacking tents that will keep you dry if you don't have equipment.  Experience tells us survey responses indicate we have a bunch more available.  Please tell us in detail what you have.  Note a typical 4 man tent is comfortable for 3 usually!   Also, do you already have a tent mate in mind? *
There are swimming opportunities associated with our camp venues.  There may be no lifeguard.    Is the athlete a safe/strong swimmer?  Do they have permission to Swim? *

In the Green Bay area we train on Rollerskis on roads that include chip seal, tar snakes, road rock and 25 mph downhills.  We require our campers have very solid rollerski skills including safety skills like grass stops and ski in High vis.  Tell us about your skills and equipment.  Desired skis are Marwe classics #6 and Swenor #2 (solid for rollerski agility!) skates. example:  "I have Swenor #2 and Marwe #6, very comfortable on hills..." *

Permission for Camp staff to arrange same gender tent assignments for camp athletes.  To agree check yes.  Camp costs are based on tent camping.   *
Permission for your athlete to drive to workout venues and also transport other athletes.  We generally will car pool and have coaches drive and avoid extra athlete trips on their own without camp staff permission.   *
