Couple definitions that may be helpful as reference:
Health Promotion is the process of enabling people to increase control over, and to improve, their health. It moves beyond a focus on individual behavior towards a wide range of social and environmental interventions.
Wellness is an active lifelong dynamic process of becoming aware of and making choices towards a healthy and fulfilling life. It is a state of complete physical, mental, spiritual, and social well-being, and not merely the absence of disease or infirmity.
Population Health is taking responsibility for managing the overall health of a defined population and being accountable for the health outcomes of that defined population. This includes leveraging data to understand the interrelated factors that influence health, identify systematic variations and disparities, and inform interventions, programs, and policies.
