Reports made with the Confidential Incident Report form will be received by Boulder Ballet’s Executive Director. Information will be kept confidential, except if the organization is required to report by law or needs to shared information to the School Director, Assistant School Director, or specific faculty in order to resolve the issue.
INCIDENT REPORT
It is the policy of Boulder Ballet (BB) to take reasonable steps to provide an environment free from unlawful discrimination and harassment, including discrimination and harassment based upon an individual’s sex, age, race, color, national origin or ancestry, creed, religion, sexual orientation, marital status, veteran status, physical or mental disability, medical condition, or any other category protected by federal, state, or local laws. As part of this policy, BB is committed to promptly investigating complaints of such behavior and taking corrective action when warranted. To assist BB in conducting an investigation of your concerns, please provide the information requested below.
All complaints will be kept confidential. Any actions regarding the complaint will be discussed with the complainant.
