Please use this form to complete Step 1 of the registration process for our Academy Division private lessons. Please ensure that all information provided is accurate before submitting.
NOTE: This form should be used ONLY for beginners ages 3-11 or intermediate or advanced students of any age.
Upon submitting this form, we will contact you with possible times to either observe a lesson (if your child has never played the instrument before) or meet with one of our faculty to audition (if your child is already playing). If you have previously studied at NJAOM, we will contact you with available lesson and class times and final steps to complete your registration.

***If you are interested in registering more than one child in the Academy Division at NJAOM, you can list each child's name by using Student 1 for the first child, Student 2 for the second child, etc. If you are only registering one child, please use Student 1 fields and leave all others blank.
