BEFORE COMPLETING THIS FORM, SUBMIT YOUR COLLEGE APPLICATION, AND, IF APPLICABLE, CHECK WITH TEACHERS THAT LETTERS OF RECOMMENDATION HAVE BEEN UPLOADED TO NAVIANCE.

- Please allow up to 2 weeks (10 school days) for transcripts (and any supporting documentation) to be submitted.
- Be aware of priority deadline dates.
- Inform your counselor when admission decisions are received (accepted, denied, wait listed or deferred).
