1. 本人/本人隊員是自願參加此活動和願意承擔自身的意外和責任，並無權向協會及大會對本人/本人隊員在往返活動場地途中、活動中發生或引致之自身意
外、死亡或任何形式的損失索償或追討責任。
I / my team members understand that by participating in the Event, there are risks of injury, death and/or loss. I am / my team members are entering the
Event at my own risk and responsibility.
I / my team members hereby discharge the HKCRA & Organizer and any other individual or organization connected directly or indirectly with the Event from
any responsibility in the event of injury, death or loss of property incurred during, as consequence of or while traveling to and from the Event.
2. 本人/本人隊員聲明本人/本人隊員身體健康及有能力參加此活動。
I / my team members declare that physically fit and capable of participating in the Event.
3. 本人/本人隊員同意協會及大會有權收集、儲存及使用本人/本人隊員在聲明上所填報的個人資料【個人資料(私隱)條例之定義】用作有關予該活動(包括籌
備、推廣或宣傳該活動等)。本人/本人隊員亦同意協會可將本人/本人隊員的個人資料發放予協會及協辦機構用作有關於該活動之用。
I / my team members agree that the HKCRA & Organizer are permitted to collect, store and use my / my team members’ personal data (as defined in the
Personal Data (Privacy) Ordinance) as provided by me / my team members in the Declaration for the purpose of or in connection with the Event (including
organization, promotion, and publicity of the Event etc.).
I / my team members further agree that the HKCRA may pass on my personal data to such of its supporting organizations for the purpose of or in connection
with the Event.
4. 本人/本人隊員明白及同意提供本人/本人隊員的身份證或護照給予協會(如有需要及在大會要求下)，以作核對個人資料。
I / my team members understand and agree to provide my / my team members’ HKID Card/Passport to verify my/ my team members’ personal derails to the
HKCRA upon request.
5. 本人/本人隊員簽署此聲明以示同意及確認所有列明之重要事項、聲明及有關細則。
