Dear Candidate for Confirmation and Parent(s)/Guardian(s):

THANK YOU for discerning, AS A FAMILY, to prepare for the Sacrament of Confirmation with St. Therese Parish!!!

This PROCESS invites you (Candidate), during your High School years, along with your Family and Sponsor, to deepen your relationship with Jesus; be His disciple with the love & support of the parish & youth community; and serve actively as an initiated member of His Body, baptized into the Church.  With the gift of the Holy Spirit, you will be strengthened to live out more fully and completely your baptismal call to "make holy the world!"

To complete registration (1 thru 5):

     1.  After prayerfully discerning our process expectations for GROUP SAUL families, please fill out
          and submit this online form.  Please set aside at least an hour of your time to do so.  (If you
          wish to have a PDF file of the entire document prior to entering your responses online, please
          send a request to rdeang@stthereseparish.org).

     2.  Attend the MEETING for Parents/Guardians AND their Children/Candidates on Sunday,
          Sept. 22, 2019, starting at 6:30 PM in the Social Center.

     3.  At this 9/22 meeting, review and sign forms WE will print for you using your online responses
          in #1 above.

     4.  At this 9/22 meeting, we request a family contribution of $150.00.  To facilitate ease of
          processing:

                    *Please prepare your CHECK--payable to "St. Therese Parish" with a MEMO of your
                      child's FULL name
                                                                 --OR--
                    *Give online prior 9/16/19 (FUND: "Confirmation" and include a MEMO of your child's
                      FULL name under "Special Intentions"):

     5.  At this 9/22 meeting, please bring and submit a copy of your child’s BAPTISM CERTIFICATE.


As you review our parish preparation process for the sacrament of Confirmation for GROUP SAUL on this online form, I recommend that both Candidate and Parent(s)/Guardian(s) do so together, taking this opportunity to discern your commitment level as a family and your teen's willingness at this point in his or her life.

BEFORE typing your responses onto this online form, please have the following on hand and ready:

     --Copy of Candidate's Baptism Certificate and (if applicable) First Eucharist/Communion
          Certificate

     --Candidate answers:  "Tell us about yourself..."

     --Candidate answers (in at least 100 words):  "What is the single biggest struggle you have when
           it comes to having a personal relationship with Jesus Christ and why?"

     --Parents/Guardians answer (in at least 100 words):  "What is the single biggest challenge you
           have when it comes to sharing your faith with your teenager(s) and why?"

     --Candidate's Medical Insurance Card/Information

     --Candidate's Emergency Contact and their contact information

     --IF KNOWN -- you can give us the Candidate's Sponsor for Confirmation and details of their
          Confirmation & contact information; AND the Saint's name the Candidate chooses (with the
          help of their Sponsor and Parent[s]/Guardian[s]) to take as their Confirmation Name
