This form is to be used for any EOA member that

1) Had an account on the old system (Club Express) but it was not transferred over to the new system ( Wild Apricot)

OR

2) A member who paid their membership on the old system (Club Express) and it is showing as expired on the new system (Wild Apricot)

OR

3) Another login related issue.
