I understand that participating in this mission trip can expose me to risks, including, but not limited to, risks of travel, illness, injury, safety and security, civil disturbances, natural disasters, working in areas with limited access to emergency services, and COVID-19.

I am voluntarily participating in the aforementioned activity and I am participating in the activity entirely at my own risk. I am aware of the risk associated with traveling to and from as well as a participating in this activity, which may include, but not limited to, physical or psychological injury, pain, suffering, illness, disfigurement, temporary or permanent disability (including paralysis), economic or emotional loss, and death. I understand that these injuries or outcomes may arise from my own or others negligence, conditions related to travel, or the condition of activity location(s). Nonetheless, I assume all related risks, both known or unknown to me, of my participation in this activity, including travel to, and from and during this activity.

If I have opted into a dual or triple-occupancy rooming assignment, I understand the risks that this may entail, as it pertains to COVID-19 transmission.

I accept responsibility for loss and/or additional expenses for extenuating circumstances.

I give permission for International Hands in Service to contact my emergency contact as deemed necessary and make emergency decisions on my behalf if I am unable to.

I agree to make available to my Mission Team Leader prior to the mission trip any pertinent personal medical information which will serve to prevent or manage a medical emergency during the mission trip.

In the event I should require medical care or treatment, I agree to be financially responsible for any cost incurred as a result of such treatment. I agree to take the precautionary measures necessary to protect my own health and safety, such as medicines, immunizations (in consultation with my primary care provider), and protection of my person and personal belongings.

I am aware I should carry my own health care insurance. I confirm that I have purchased a travel insurance policy.

I will adhere to International Hands in Service policies 1) stipulated in the Mission Journey Guidebook (https://www.saludypaz.org/get-involved/teams/mission-team-resources/) and 2) others as communicated to me by International Hands in Service staff.

I agree to respect the authority of International Hands in Service staff throughout the mission trip.

I understand that International Hands in Service takes no responsibility for, nor can endorse, any activities outside of the mission itinerary.

I give permission for International Hands in Service to use photos and videos taken during the mission.

I agree to indemnify hold harmless International Hands in Services Directors, Officers and Employees against any claims, suits, or actions of any kind whatsoever for liability, damages, compensation or otherwise brought by me or anyone on my behalf, including attorney’s fees and any related cost, if litigation arises pursuant to any claims made by me or by anyone else acting on my behalf.

I acknowledge that this activity may involve a test of a person’s physical and mental limits and may carry with it the potential for death, serious injury, and property loss. The risk may include, but are not limited to, those caused by terrain, facilities, temperature, weather, lack of hydration, condition of participants, equipment, vehicular traffic and actions of others, including but not limited to, participants, volunteers, spectators, event officials and event monitors, and procedures of the event.

I acknowledge that I have carefully read this “waiver and release” and fully understand that it is a release of liability. I expressly agree to release and discharge International Hands in Service and all of its affiliates, manager, members, agents, attorneys, staff, volunteers, heirs, representative, predecessors, successors, and assigns, from any and all claims or causes of action and I agree to voluntarily give up or waive any right that I otherwise have to bring a legal action against International Hands in Service for personal injury or property damage.
