We will use this form to enrol your child. It is crucial therefore that the information returned to us is accurate and punctual so that we can communicate with you effectively and proceed without delays.  

The form should take around 10 minutes to complete. In order to complete the form fully you will need the following ready:

- the details, including addresses, emails and phone numbers, of three contacts for your child
- the details of your child's doctor/surgery
- you will also need your child for Section 10. This portion of the form needs to take into account some of their views
