Note: Accomplishing this form is one of the requirement to process your application for possible candidacy for TESDA TWSP (Training For Work Scholarship Program) / STEP (Special Training for Employment Program).  Please answer each question/statement or select your choice as honestly as possible. Your response will help Asian College, TESDA Tech-Voc Dept. to able in processing your scholarship application.

Scholarship Requirements:
Scholarship is Open for NCR, Metro Manila only.
These are the following mandatory requirements needed in order for you to get a slot in the said scholarship

• Age requirement: 18 yrs to 55 years old
• FOR HIGH SCHOOL GRADUATE:
               Photocopy of Form 137
               HS Diploma
• FOR COLLEGE UNDERGRADUATE/LEVEL:
               Photocopy of Copy of Grades/Units
               HS Diploma
• FOR COLLEGE GRADUATE:
               Photocopy of Transcript of Records
               Diploma
• PHOTOCOPY OF PSA BIRTH CERTIFICATE
• 4 PHOTOCOPIES OF ONE (1) VALID GOV'T./SCHOOL I.D.
• (for Female) PHOTOCOPY OF MARRIAGE CERTIFICATE
• 4 PCS. 1x1 I.D. PICTURES
• 6 PCS. PASSPORT SIZE I.D. PICTURES
          (NOTE: required strictly with white background, with collar, with name tag (full name including middle initial) and in matt finish)
• PHOTOCOPY OF VACCINATION CARD
• All scholar applicants should be fully vaccinated

Failure to submit all requirements will automatically reject your scholarship application slot.

Online classes would require scholars to have:
• smartphone / tablet / desktop / laptop
• proper internet / WIFI / broadband connection / mobile data

Required to have and need to be knowledgeable in using these applications:  
• Gmail acct (email address)
• Google meet
• Google classroom
• Google apps device policy (for mobile access in using google classroom)
• Google docs and sheets
• GCash (account with your real proper: name, birthday, email, number and address) for allowance purposes
