PPVIC Mentorship Programme 2022-2023 Application Form (for Mentors)

Before filling this form, please take some time to have a look to the “Program Objective” , “Role of Mentor” and “Program Features” below:

Program Objectives: 
Inspire mentees and broaden their life horizon by sharing mentors’ stories and experiences.
Accompany mentees to explore their life planning and assist in enhancing their sense of hope for their future.
 
Role of Mentor:  
  To assist the mentees in understanding themselves, exploring life planning, developing positive thinking, broadening the horizons of life and enhancing the sense of hope.

Program Features:
One/two mentees will be assigned to each mentor.
Mentors make good use of their personal resources and networks that are obtained from their work and life experience, to assist the mentee to develop positive thinking and provide advice to mentees in respect of personal development, life planning, career and academic choices.
The programme is planned to be from November to June of the following year for a period of at least eight months. During this period, the mentor and the mentee need to meet at least four times.   After the program is completed, the mentor and the mentee can choose whether to continue the mentorship relationship or not.

Part B: Mentor’s Declaration

I, [Full Name of Mentor] _________________________________, hereby apply for admission to the Pope Paul VI College Mentorship Programme 2022-2023. I understand, declare and agree that:

  1. All application details above are true and correct.
  2. Depending on the number of applications received, I may not be accepted to the programme.
  3. If admitted to the programme, I will fully commit myself to the programme and reserve the specified dates to participate in the Opening Ceremony, Main Mentorship Activity 1 and the Closing ceremony.
  4. The decision of the PPVIC Mentorship Programme Committee on any matters arising out of the programme will be final.
  5. I will treasure the precious opportunity to provide guidance for my mentees and to contribute constructively to my alma mater.
  6. I understand that aside from the three official events organized by the PPVIC Mentorship Programme, I may initiate small group gathering(s) for my mentees group. If I do so, I must ensure that consent is gained from the parent(s) / guardian(s) of my mentees. The parent(s) / guardian(s) and the mentee concerned are fully liable for any safety issues that arise from the gatherings.
  7. This is purely an educational program and no commercial activities are intended or allowed. I understand that as a mentor, I am not supposed to be selling or offering to sell anything (whether in the form of my or my company’s or employer’s, products or services) to, or soliciting anything (money or otherwise) from, my mentees or their families. The mentees will report to the Careers Mistress of Pope Paul VI College in writing as soon as practicable if any of the above were to happen. I further understand that Pope Paul VI College has not authorized any such offers or solicitations by me and that if my mentees (or their families) choose to accept such offers or make any payments, they do so at their own risks and the school shall not be responsible for any consequence or loss suffered.
  8. I will complete an evaluation form of the Pope Paul VI College Mentorship Programme upon the completion of the programme.
  9. Pope Paul VI College, the Alumnae Association, PPVIC Mentorship Programme Committee and the mentees accept no liability whatsoever for any injury, loss and damages that I may suffer as a result of my participation in the Pope Paul VI College Mentorship Program 2022-2023, and I hereby waive all my rights against them.
  10. Personal Information Collection Statement:
    Your personal data collected in this form ("the data") will be used by the PPVIC AA or the Careers Mistress of Pope Paul VI College for the purposes of proceeding applications for Pope Paul VI College Mentorship Programme.  Personal data collected will only be transferred to such persons who are responsible for carrying out the above purposes and be kept as record. You have a right to request the access to and correction of the data. Any such request should be addressed to mentorship@ppaulvi.edu.hk
