Covid-19 Positive Reporting Form
Thank you for using the Woodland Park Academy Covid-19 Positive Reporting Form.

Please fill out a form for each of your students that have tested positive for Covid-19.

Your student must isolate for 5 days after testing positive, and remain home from school.  Your student may return to school after 5 days if their symptoms are resolving without the management of medication.
 
Symptoms are: (cough, fever, shortness of breath, sore throat, muscle pain/body aches, loss of taste or smell, headache, diarrhea, vomiting, abdominal pain, congestion or runny nose).

If you have additional WPA students in the home that have not tested positive and are symptom-free, please call the office at (810) 695-4710 to determine if the WPA Test-to-Stay Program is an option for your family.
