Student-Athlete Participation Waiver WAIVER/RELEASE FORM.

I realize that Sports Training is a vigorous physical activity that involves weight lifting, jumping running and rapid directional change. I understand that participation in the Private vs Public All Star Game /Training involves certain inherent risk and that regardless of the precautions taken by TRU Sports Foundation or the participants, some injuries may occur. These injuries may include, but are not limited to: Hamstring, shoulder, wrist, ankle and knee injuries These injuries may result from hazards such as:

1. Not being properly stretched

2. Not protecting areas by being taped (ankles, wrist) I further certify that my present level of physical condition is consistent with the demands of active participation in Sports Training.

Following is a full and complete list of all of my known health conditions that might affect my ability to participate: I have carefully read the foregoing document. I am confident that I fully know, understand, and appreciate the risk involved in active participation in the Private vs Public All Star Game.

Having been informed of the above program to provide sport training opportunities for participants, I, the parent of the above-named registrant, do hereby give my approval of his/her participation in any and all of the activities during the duration of the Event. I assume all risk and hazards incidental to the conduct of the activities, and I do further release, absolve, indemnify, and hold harmless TRU Sports Foundation, the organizers, sponsors, supervisors, volunteers, and officials, or any or all of them. In case of injury to my son/daughter, I hereby waive all claims against the organizers, the sponsors, or any of the supervisors appointed by them. I am voluntarily requesting permission for my son/daughter to participate. We also understand that TRU Sports Foundation retains the rights to use for publicity and advertising purposes, photographs of players taken during practice and/or games.
