Guest Administration Approval Form (To be completed by the Visiting Guest's Administrator)
TO BE COMPLETED BY AN ADMINISTRATOR AT THE GUEST’S HIGH SCHOOL: a principal, activities director, or dean.
1)  The student (MHS or Guest) emails this link to the guest's high school administration (principal, assistant/vice principal, activities director, or dean).
2)  The guest's administrator will complete this form no later than Thursday, February 8, end of day.
3)  The guest must be a current high school student in good standing.
4)  Mr. Schultz (MHS Activities Director) will confirm this form when completed and email the administrator at the address provided.  Any invalid email address will lead to both students NOT attending, with NO refund.
5) The request for an email address in this form refers to the administrator's email.
