This Waiver is to request approval for a Combined Town Program Select Team,. This is when 2 towns EACH do not have enough players to form their own Select Team.  
This waiver must be submitted, and approved, before you register your team. Previous year's approval does NOT guarantee approval for 2024. Please only submit 1 Waiver Request per team.
