BOSS Absence Request Form
Please complete the form below and provide a brief description of your absence. If you are submitting an absence for siblings, please ensure that each athlete has a separate form completed for their absence (one per athlete). It is at the admin and owners discretion whether your absence request will be considered approved or denied. You will receive an e-mail within 1-2 business days of submitting your request to notify you if your absence will be approved or not. All absences must be submitted 48 hours in advance (with the exception of family emergencies). All absences without request will result in athlete probation.
