This survey requires a commitment from the parent or guardian on whether or not you will opt for your child/ren the full remote learning instruction or face to face instruction. Please understand that once you state that you want your child/ren remote learning instruction, you cannot opt for in-person instruction until the end of the 1st Trimester, November 30, 2020. If then you opt for in-person instruction, proper protocols must be followed.

Families will be given 3 days from the release of this survey to make their decision. Survey must be filled-out with electronic signature and date by Friday, August 14, 2020, so that we may make proper arrangements

St. Aloysius Elemtary Academy is planning to offer two options for school in the fall. Please choose the option that best meets your family’s needs, if a remote option is chosen, students will not be permitted in school until The beginning of the second trimester on November 30, 2020. A request must be submitted to the principal with a negative COVID-19 test result: *
For In Person Option, all students will be required to wear masks at all times. Please indicate your acknowledgement. *

Would you be receiving lunch from the school or sending lunch from home: *
For In Person Option, there will be a health assessment (temperature check) at the entrances and periodically throughout the day for all students. Please indicate your feelings regarding this requirement. *
For In Person Option, the school has experienced situations where students are given fever reducers prior to coming to school. This action has likely caused other students to become ill, and additional safeguards will be in place to strongly discourage this from happening in the future. Please write in how you believe the school should respond to situations where students are found to have taken fever reducers to mask symptoms: *
Regarding Aftercare, if parents reserve the space on a daily basis, they will be billed in advance since another student has been denied the space. These new procedures will be detailed in a new Aftercare document to be issued in the near future. Please indicate your response: *
For Distance Learning Option, please indicate your agreement that you may NOT switch to In Person Option until the beginning of the 2nd Trimester. *
For the Distance Learning option, parents are cautioned that an agreement will be required detailing expectations regarding student attendance, participation, and timeliness of work submissions. Please indicate whether you will be able to assist and supervise your child with their schoolwork, especially if it is done outside of school operating hours. *
