This form is only for the PERSON to register as a member, for food and presentations, sign the waiver, and provide payment instructions.

Show Entry Fees: (this in inclusive of the presentation fees)
$30 per entry
$25 for 4 or more entries by the same owner or co-owner
$15 Baby Puppy
$10 Show Catalogue

Presentation Fee: (only needed if you are NOT entering into the showcase with a dog)
$20 per presentation or $30 to attend both

If you have previously provided proof of registration for a dog you will be showing again no need to send again.
