BPB Tour Reservation Form (2022)
THIS FORM IS FOR OUR PRIVATE, FAMILY AND SMALL GROUP TOUR PROGRAM FOR UP TO 10 GUESTS IN A GROUP.

Please complete this form prior to your our date. If you have not already secured a date, a link to the booking calendar can be found at the end of this form!

Immediately upon booking you will get an email with links to the release and links to this form to be sure all guest have completed it.

Approximately 48 hours prior to your tour, you will receive and email with the check in instructions.

You will also get automated reminders approximately 24 hours before your tour and again 2 hours before.

**DOES ANYONE IN YOUR GROUP HAVE A PEANUT ALLEGY?**We typically feed peanuts to our pigs. If anyone in your group has an allergy to peanuts, we will substitute another, safer treat to feed! *

How many children are in your group? Please include ages *PLEASE NOTE:  CHILDREN ONE YEAR OF AGE AND OLDER ARE CONSIDERED IN THE GUEST COUNT. We love names so we can greet them personally OR feel free to introduce the group when you arrive. *
How many adults are in your group? *
If you have special accommodation requests, such as handicapped or stroller assistance, masks on the entire time, etc.,  please note them here. If you have someone in your party with any special concerns, like being shy or afraid of animals, please let us know so we can help! If someone is celebrating a birthday, please let us know as well! If there is anything we can do to make your tour more enjoyable, we want to know. We love to say HAPPY BIRTHDAY so if someone is celebrating, please let us know! *
