This is the sign-up and payment form for the "Introduction to Fly Fishing" class hosted by the Fishing Museum and Cultural Center! The class is held at Millennium Park in Grand Rapids (with the possibility of other nearby parks) and meets Tuesdays in May; 5/4, 5/11, 5/18, 5/25 from 6:00 pm to 8:00 pm. The cost is $80 per student. All equipment is provided (personal equipment is allowed with prior permission). All students must follow Michigan DNR fishing regulations. This sign-up is for one individual. If there are more people in your group, each person will have to fill out a form. This class is full of education, practice, and fishing!
